IISER-K Contributory Medical Scheme (IISER-K CMS)

  1. The Scheme
  2. IISER-K, as part of its commitment to employee welfare, operates the IISER-K CMS which covers all   the employees (in service and retirement) and members of their families, employees appointed on   temporary basis as well as staff working in research projects for a period exceeding three months, students, Emeritus Scientists, Professors of Endowed Chairs, Senior Associates, Visiting Scientists / Fellows, and Research Fellows. The Scheme came into operation on 8 April 2008. [Copy of the CMS Rules may be found in the libraries at Mohanpur and Salt Lake]

  3. How to join the Scheme
  4. Monthly Contribution: To avail of the medical benefit,   a monthly contribution at the following rates needs be made by the beneficiary:  1) Employees -- One percent (1%) of the basic pay subject to a maximum of Rs. 225/- , and 2) Students -- Rs. 50/-.

    Annual Declaration: All employees should, while joining the Institute and at the beginning of each calendar year, declare on prescribed   Declaration Form information on dependents. In cases of chronic diseases a separate declaration to that end too should be made.

    Stream of Medicine: Treatment may be availed of from any stream of medicines - allopathy, homeopathy, ayurveda etc

  5. Treatment
    1. Indoor Treatment/ Hospitalization
      1. Treatment (indoor) in Empanelled Hospitals/ Nursing Homes:  The treatment will be cashless.  The hospital concerned will send the bills directly to the Institute for payment. In cases of emergency, patients may be admitted based on production of office ID cards of the beneficiary or telephone call from the Chief Medical Officer (CMO). In any case, the CMO or Medical Unit should be informed in writing of the admission of the patient based  on which    official letters will be issued by the Institute.  [These hospitals/nursing homes are expected, as per agreement under CMS, to bill according to the approved rates]
      2. Treatment in Non-empanelled Hospitals/ Nursing Homes: The CMO or Medical Unit should be informed in writing of the admission of the patient within 24 hours.
      3. Beneficiaries should settle the bills for any consultation / treatment directly with such hospitals / nursing homes and seek reimbursement from the Institute in Claims Form (alongwith the Check Sheet). The reimbursement in such cases will be restricted to the approved rates.

      4. Examinations for Diagnosis: Such tests can be carried out at the Empanelled Diagnostic Centres or any other non-empanelled centres. The Empanelled Centres are bound, as per the agreement under CMS, to charge at the approved rates only. Those who choose to avail of service at the Empanelled Diagnostic Centres should produce their ID card at the Centre concerned for identification and billing accordingly. In all cases the reimbursement will be restricted to the approved rates.

    2. Out-door treatment

    The patients may consult Family Physicians (including Gynecologists, Pediatricians) as given in their Declarations. Specialists may be consulted on referral by the Family Physician / Chief Medical Officer and those from the panel of specialists prepared by the Institute.

    Reimbursement of claims may be made in the Claims Form (alongwith Check sheet) based on the approved Consultation Fees.

  6. How to make Claims for Reimbursement of Medical Expenses
  7. Claims for reimbursement of medical expenses  should be made in the Claims Form (alongwith Check sheet) based on the Consultation Fees or CGHS Rates as the case is. The filled-in forms should be deposited in the Medical Drop Box placed at the Reception at Mohanpur Campus. These will be cleared weekly and processed for payment on monthly basis.

    Late submission of medical bills is not allowed. In case of exigencies the beneficiaries concerned should enclose a prayer, with the bills, for condonation to the Chairman of the Medical Committee furnishing thereby satisfactory explanation for the delay. [Bills submitted after three months of any consultation will be considered as late submission.]
